Oxiracetam

Oxiracetam

Oxiracetam is a synthetic racetam with mixed, population-specific human evidence, not an established healthy-adult memory or focus aid. A large oral post-stroke trial found no cognitive advantage over placebo; newer brain-injury research used intravenous products and a distinct L-enantiomer. Compare oral powders by identity, net mass and price per gram, keeping study exposure separate from a personal dose.

Extra caution

Who should avoid this or get expert review first

This is not a recommendation to use. These groups should treat the profile as a reason to slow down and verify safety, rules, and personal context.

  • Groups outside the base-risk scope8The oral label contraindicates severe kidney impairment and allergy to the product, advises caution with lesser kidney impairment, and says it should not be used during pregnancy or breastfeeding. Pediatric safety and efficacy are unestablished.
01

What is it?

Read the population and route first4
A study in dementia, stroke or brain injury answers a different question from improving normal study or work performance. Oral racemic oxiracetam, the L-enantiomer and intravenous formulations should not share a single benefit or dose claim.

Identity matters

1

Oxiracetam, also known by the research code ISF-2522, is a synthetic racetam. Its chemical identity is distinct from piracetam, and an identifier in a substance database is not approval of a finished consumer product.

Older positive findings were small

2

A 1992 placebo-controlled study enrolled 65 older adults with degenerative, multi-infarct or mixed dementia. Some cognitive tests and a quality-of-life scale favored oxiracetam over 12 weeks. Two participants receiving oxiracetam withdrew for poor tolerability. This is condition-specific evidence, not a healthy-adult performance result.

A conflicting Alzheimer study

3

Another double-blind study in 24 patients with probable Alzheimer's disease found no cognitive improvement on its neuropsychological battery. The conflict with positive small studies argues against a general memory-enhancement claim.

The brain-injury trial is now complete

10

The 2025 LOCATE trial enrolled 590 people after mild-to-moderate brain injury. It compared intravenous L-oxiracetam, racemic oxiracetam and placebo for 14 days. L-oxiracetam favored the primary cognitive score at day 90 versus both comparators, but many secondary outcomes were null and about one quarter did not complete that assessment. This is not evidence for an equivalent oral powder regimen.

A larger oral trial found no cognitive advantage

9

In a 2025 post-stroke trial, 500 participants were randomized and 457 completed follow-up. Neither the MMSE nor CDR-SB cognitive endpoint favored oxiracetam. Exploratory imaging findings did not overturn those null results. Adverse events were numerically more frequent with oxiracetam, but the difference was not statistically significant and investigators identified no clinically relevant attributable signal.

Mechanisms of action

Mechanistic proposals remain indirect

4

Racetam research discusses cholinergic signaling and neuronal membrane processes, but much of the mechanistic work is preclinical. A plausible pathway cannot establish a useful cognitive effect, a personal dose or superiority over another racetam.

04

Potential effects and evidence map

Grade C reflects inconsistent human cognitive findings and limited relevance to healthy-adult enhancement. Older small dementia studies conflict, a large oral post-stroke trial was negative, and recent positive brain-injury findings used intravenous preparations. Neither a racetam label nor measurable blood exposure proves better everyday cognition.

Reliable focus or productivity improvement in healthy adults is not established.

Note Patient outcomes and preclinical mechanisms do not establish an everyday performance benefit.

Typical dosages and timing

Studied daily amount91600 mg/day

The study used 800 mg twice daily for 36 weeks and found no advantage on either cognitive coprimary outcome. This is research context, not a validated dose for healthy-adult enhancement or a suggested cycle.

Plasma timing is not an effect clock5

A small single-dose study in eight healthy Japanese volunteers found plasma peaks around one hour and terminal half-lives of 5.7-9.8 hours across fasting dose groups. Only six received active drug. These measurements do not establish when better cognition starts, how long it lasts or when to redose.

07

Safety, side effects and risk profile

Why the base rating is low8
For identified oral oxiracetam at studied exposures, low reflects reactions that can limit use: the regulator calls for dose reduction with excitation or sleep disturbance. Minimal would understate that signal. High is not justified by research labeling or uncertain benefit. The short healthy-volunteer studies cannot define long-term risk.
Legal / regulatory risk11
Confidence: High

Rules depend on jurisdiction and product category.

Note The historical U.S. enforcement case does not establish a global ban.

2/5
B
Evidence uncertainty4
Confidence: High

Healthy-user efficacy and long-term safety remain incompletely characterized.

Note This uncertainty does not itself raise base risk.

1/5
C
Interaction risk4
Confidence: Low

The cited trials do not establish safe consumer stacks.

Note An absence of documented interactions is not a compatibility guarantee.

1/5
D
Headache / dizziness8
Confidence: Medium

Oral postmarketing reports include dizziness, tremor and seizure-like events.

Note Frequency and healthy-user applicability are not quantified.

2/5
C
GI discomfort6
Confidence: Low

A tiny repeated-dose study reported mild, transient constipation.

Note No substantial vital-sign or laboratory changes were observed.

1/5
C
Overstimulation / insomnia8
Confidence: Medium

The oral label calls for dose reduction with excitation or sleep disturbance.

Note This is a potentially dose-limiting reaction, not an enhancement claim.

2/5
B
Pregnancy / lactation caution8
Confidence: High

The oral label says pregnancy and breastfeeding use should be avoided.

Note Safety in these populations is not established.

3/5
B
Liver / kidney caution7
Confidence: High

Kidney impairment can substantially prolong elimination.

Note This targeted caution is not evidence of common kidney injury in healthy users.

2/5
B
Reported events have important limits8

China's oral-label update includes allergy-like reactions, seizure-like events, arrhythmias and abnormal liver or kidney function among postmarketing reports. It gives no incidence or healthy-user causal estimate. Injection-only shock warnings should not be transferred to an oral powder.

08

Interactions and cautions

Stack evidence is missing4

The cited literature does not validate combining oxiracetam with other racetams, stimulants or choline products. A proposed cholinergic mechanism is not evidence that choline is required, and sparse interaction reporting does not establish compatibility with prescription medicines.

10

How it compares

Form and route check10

The L-enantiomer is one stereochemical component of racemic oxiracetam. The recent clinical comparison used different intravenous preparations and doses under inpatient care. Do not divide a trial dose or copy its results onto an unspecified retail powder; verify the actual identity and route first.

11

Practical buying and quality notes

  • For single-ingredient powder, compare the package total and price per gram. Fifty grams is 50,000 mg of listed powder, not 50,000 mg per serving. Capsules require their declared oxiracetam amount as well as the count; sample packs and blends are not equivalent units.1
  • Check whether the certificate identifies oxiracetam, matches the package lot, names the analytical method and reports relevant impurities. The 2022 U.S. enforcement case included misleading testing claims; it is not evidence of misconduct by today's listed vendors. A purity percentage cannot establish clinical suitability.11

FAQ

What oxiracetam dose is supported for healthy adults?

9

No dose is validated for everyday cognitive enhancement in healthy adults. The displayed daily amount belongs to a clinical study with negative primary results. Trial dosing is not a powder-measuring instruction, and increasing exposure does not establish greater benefit.

How long does oxiracetam last, and should it be cycled?

7

Plasma elimination is measurable, but a useful period of enhanced cognition and an evidence-based cycling schedule are not established. The study window is not a recommended cycle. Kidney function also changes elimination, so an average The time for blood levels to fall by half during the final elimination phase.Source cannot specify a personal redosing interval.
