Pramiracetam

Pramiracetam

Pramiracetam is a synthetic racetam with limited, older human evidence. Small impairment and drug-challenge studies do not establish everyday focus or memory benefits in healthy adults. Research doses and plasma timing are context, not a personal regimen. Compare powders and tablets only after checking the chemical form, declared pramiracetam amount and local medicine rules.

Extra caution

Who should avoid this or get expert review first

This is not a recommendation to use. These groups should treat the profile as a reason to slow down and verify safety, rules, and personal context.

  • Pregnancy, breastfeeding and organ impairment8The official Pramistar information says not to use it during pregnancy or breastfeeding, with severe kidney impairment, or with hepatic impairment. Mild-to-moderate kidney dysfunction requires caution because elimination slows.
01

What is it?

What the evidence can answer6
Pramiracetam has human pharmacokinetic and small cognitive-study records, but a drug-challenge result is not a demonstration of better everyday memory. The useful comparison is between the actual preparation, evidence and safety conditions, not a marketing claim that one racetam is a certain number of times stronger.

Identity and formulation

1

Pramiracetam, developed as CI-879, belongs to the synthetic racetam family. Free-base and salt identifiers do not imply identical masses. Pramistar is a medicine brand, not proof that every retail powder with the same ingredient name is an equivalent product.

A small acquired-injury study

5

An older double-blind placebo-controlled study in males with memory problems after head injury or anoxia reported better memory performance, especially delayed recall. Its later 18-month phase was open-label, followed by one month off drug. That follow-up is not proof of a lasting benefit for healthy adults, and the very small clinical sample limits generalization.

Healthy volunteers under a drug challenge

9

A randomized study included 24 men in two age groups. After 10 days of pramiracetam or placebo, scopolamine was used to impair memory experimentally. Pramiracetam partly reduced the induced amnesia. This design asks about resistance to a drug challenge, not better baseline cognition, and the abstract does not quantify adverse events.

A largely negative Alzheimer's trial

10

In 10 patients with probable Alzheimer's disease, eight showed an apparent best dose during dose-finding, but only two improved comparably in the replication phase. The investigators concluded that doses up to 4000 mg were unlikely to provide symptomatic benefit. This negative result cautions against a general claim that higher exposure means better cognition.

Mechanisms of action

Cholinergic hypotheses are not a benefit guarantee

6

Pramiracetam is discussed in relation to cholinergic signaling and choline handling, but its mechanism is not fully established. Mechanistic or animal findings cannot specify a useful human cognitive dose or prove that adding choline makes it work.

04

Potential effects and evidence map

Grade C reflects small human studies in brain injury and experimentally induced memory impairment, with mixed or negative findings in other patient research. These populations and designs do not establish useful enhancement of normal cognition. Pharmacokinetic measurements show drug exposure, not benefit.

Effect domainMagnitudeGrade
Memory / learning9
UnknownConfidence: Low

Limited memory findings occur in specific impairment models, not established normal-cognition enhancement.

Note The scopolamine experiment partially reduced induced amnesia; the small brain-injury study is not representative of healthy consumers.

C
Focus / attention9
UnknownConfidence: Low

Reliable focus or productivity improvement without induced impairment is not established.

Note Challenge-model results cannot be translated into a routine study or work benefit.

1/5
D
Mental energy / wakefulness6
UnknownConfidence: Low

The cited human studies do not establish a wakefulness benefit.

Note Drug exposure and a nootropic classification are not demonstrations of useful mental energy.

Typical dosages and timing

Studied daily amount91200 mg/day

The drug-challenge experiment used 600 mg twice daily for 10 days before the challenge. This research exposure is not a recommended healthy-adult dose, a powder-measuring instruction or evidence that larger doses work better.

Plasma measurements, not a focus timer4

The 1985 study gave 12 fasting healthy men single oral doses from 400 to 1600 mg. Plasma peaks occurred at 2-3 hours; harmonic mean elimination half-lives across dose groups were 4.5-6.5 hours. No significant side effects were observed in that small experiment. These values do not establish felt onset, cognitive-effect duration, a safe redosing interval or long-term safety.

07

Safety, side effects and risk profile

Why the base rating is low, not minimal8
For oral pramiracetam at researched or medicine-label exposures, dizziness, agitation and insomnia can affect functioning and make minimal too reassuring. The available evidence does not establish frequent severe harm in generally healthy adults, so high is not justified merely by legal restrictions or research-product labeling. Patient-label reaction rates are not exact healthy-user probabilities; long-term self-directed use remains poorly characterized.
Legal / regulatory risk3
Confidence: High

Country-specific medicine records do not establish unrestricted retail or import eligibility.

Note As checked October 12, 2026, EMA lists national products in three EU states; U.S. orphan designation is not approval.

2/5
B
Evidence uncertainty6
Confidence: High

Benefit and long-term healthy-user evidence remain limited.

Note Uncertainty alone does not determine the base-risk rating.

1/5
C
Interaction risk7
Confidence: Medium

Bleeding-risk and thyroid-medicine combinations need individual review.

Note The medicine label bases important cautions on related-drug data, not a proven interaction with every medicine.

2/5
C
Headache / dizziness8
Confidence: Medium

Dizziness is a common labelled reaction and can interfere with driving.

Note The patient leaflet also notes tremor and confusion; do not drive while affected.

2/5
B
GI discomfort8
Confidence: Medium

Nausea and upper-abdominal pain are common labelled adverse effects.

Note These are tolerability concerns, not evidence that severe toxicity is common.

1/5
B
Overstimulation / insomnia8
Confidence: Medium

Agitation and insomnia are common labelled reactions.

Note These can meaningfully disrupt sleep or functioning at medicine-label exposure.

2/5
B
Pregnancy / lactation caution8
Confidence: High

The cited medicine label contraindicates pregnancy and breastfeeding.

Note This is a separate population-specific restriction.

3/5
B
Liver / kidney caution7
Confidence: High

Severe renal impairment and hepatic impairment are contraindications in the cited medicine label.

Note Kidney impairment can slow elimination; this is not a claim of common liver injury in healthy users.

3/5
B
Labelled tolerability findings8

The official patient leaflet lists dizziness, agitation, insomnia, nausea and upper-abdominal pain as common. Confusion and tremor are less common. These can affect daily activities even when not medically serious.

08

Interactions and cautions

Direct evidence versus class cautions7

The Pramistar label reports no demonstrated interactions with several studied medicine classes, but still cautions about anticoagulants, antiplatelets and bleeding disorders based on piracetam-class findings. Its thyroid-extract warning also comes from piracetam, not direct pramiracetam proof. These distinctions do not establish that an arbitrary stack is safe.

10

How it compares

Form and label check7

The cited Pramistar tablet declares 600 mg pramiracetam supplied as its sulfate salt. A powder listing should specify whether its grams describe free base, total salt or active-equivalent content. Compare only the same declared basis; do not apply a tablet count or medicine dose directly to an unspecified powder.

11

Practical buying and quality notes

  • Use price per gram for a powder, and declared pramiracetam amount rather than tablet or capsule count alone for unit products. A medicine tablet, a single-ingredient research powder and a multi-ingredient blend are not interchangeable purchase categories. Check salt identity, region and shipping before comparing their totals.7
  • Check whether a certificate matches the lot, identifies the chemical form and assay method, and reports relevant contaminants. A purity percentage without identity or batch information is weak evidence. Testing does not establish clinical suitability, and a research label is not proof of medicine approval.11

FAQ

What is the evidence-based pramiracetam dose?

9

There is no validated dose for everyday cognitive enhancement in healthy adults. The displayed research amount belongs to a small drug-challenge experiment, not a general supplement recommendation. A prescription product follows its own local label and clinical context.

How long does pramiracetam last, and should it be cycled?

4

Plasma The time for blood levels to fall by half during the final elimination phase.Source describes elimination, not a proven period of improved focus or memory. Human research does not define a useful healthy-user effect duration, a redosing schedule or a cycling protocol. The featured 10-day study window is exposure before a laboratory challenge, not a suggested cycle.
